Coordinated Assessment Process and Education, Health and Care (EHC) Plan Information Pack & Resources 

Education, Health and Care (EHC) plans have replaced the Statement of SEN and Learning Difficulty Assessment (LDA). No new Statements of SEN or LDAs will be offered from 1 September 2014. The statutory assessment process must be co-ordinated across education, health and care. It is vital that the EHC plans reflect the views, interests and aspirations of children, young people, and their parents. Information relating to transitional arrangements can be found in the Department for Education's SEND Implementation Pack by clicking here.

Version 6 (March 2015) of the 0-25 Coordinated Assessment Process and Education, Health and Care (EHC) Plan Information Pack has been updated and draws together helpful resources, case studies and learning from implementation of the SEND reforms to date. Version 3 published in December 2013, Version 4 published in April 2014 and Version 5, October 2014 are also available.
